DOCUMENTS PROVIDED BY ENTRANTS IN SELECTION COMMITTEES
College entrants submit the following documents to a selection committee:
- a statement to the head of the technical college (to the head of the university - to enroll in college, which is a structural unit of the university) in the prescribed form;
- original certificate of education and its annexes;
- originals of certificates of the centralized testing (the Belarusian (Russian) language, biology), carried out in the Republic of Belarus in a year of reception;
- medical health certificate in the form prescribed by the Ministry of Health, and its copy;
- the documents confirming the right of the entrant on privileges at reception on studying (the certified copy of documents);
- 6 photos of 3 x 4 cm
The following documents are attached:
- 2 envelopes with stamps;
- Cardboard box files
In addition to these documents, provided if necessary :
- the conclusion of a medical and consulting or medico-rehabilitation commission of experts about the absence of contra-indications for studying on the chosen specialty (for the persons which have finished establishments providing special education for disabled children under the age of 18, disabled of the group I, II and III );
- the conclusion of the state center correction and development of training and rehabilitation of the recommendation learning institutions providing special education (for those with impaired vision, hearing, and the musculoskeletal system);
- the reference of rural executive committee about constant (not less than two years) accommodation at the moment of education in the rural settlement or in the settlement located in the territory of radioactive pollution (for the graduates of establishments of the education located in specified settlements);
- the documents confirming the Belarusian nationality (for the Belarusians being citizens of the foreign states or persons without nationality, constantly living in the territory of the foreign states);
- the agreement between the applicant and the customer on a specific training (for the entrants participating in competition on target places).
